APTT clot waveform analysis in COVID-19

COVID-19患者では，凝固異常が合併しやすいことが知られている．私たちは，ACL-TOP 700 hemostasis testing systemにて，APTT凝固波形解析をしたところ，COVID-19患者では，高率に凝固波形パターン異常がみられることを経験した．その後の報告をみると，当該測定系以外の測定系では，このような波形パターン異常は報告されておらず，測定系特異的な現象と考えられる．一方で，凝固塊の形成速度，加速度を示す指標は，調べた範囲のすべての測定系で上昇しており，COVID-19での凝固異常をAPTT凝固波形解析は反映していると考えられる．その機序としては，現在のところ特定できていないが，凝固波形パターン異常を示す検体では，特にフィブリノゲンが高値を示していた．その他，ループスアンチコアグラント，VWF/ADAMTS13，血中脂質の変動などが要因として考えられる．一方で，その重症度，易血栓性との関連など臨床的な意義はまだ不明であり，今後の症例の蓄積が必要と考えられる．
